Hospital ID pass

From 1st February 2015 you will need a Hospital ID pass for your visit to the Bronovo outpatient’ clinics. The pass replaces the blue registration card.

This has advantages for you and for us. The introduction of a Hospital ID means:

  • That we can provide a better and quicker service
  • That all data, necessary for optimal care, are together on one card
  • An improvement for the security of patient data

Good to know: the making of the Hospital ID Passes may temporarily cause more delay at the registration desk in the lobby. If you have an appointment, we ask you to come earlier than usual, so that you can get your new Hospital ID. We apologise for any inconvenience.

How to get a Hospital ID

The Registration Desk in the lobby issues the Hospital ID passes. For your new Hospital ID, we ask you to bring a valid identification document and health card with you.

Valid identification documents are:

  • A passport
  • A identity card
  • A driving licence
  • A residence permit

Tip: The registration is quicker if you register with a passport or identity card.

What information is included on a Hospital ID?

The Hospital ID includes:

  • your initials and family name
  • your social security number,
  • your Bronovo patient number,
  • date of birth and gender (If you are part of a multiple birth, the symbol II is displayed after the gender)
  • the expiry date of the card,
  • your personal barcode
  • and your photo.

The patient registration system uses the address that is registered in the municipal personal records database (GBA). If you have had a change in your address, you can arrange this through your local council.

The photo on your Hospital ID will be picked up from your passport or ID card via the chip. This is the quickest. If you register with your driver's licence or residence permit a photo is made with our webcam.

What can I do with my Hospital ID?

Your Hospital ID is an important pass. The card contains all the information Bronovo needs to have when making appointments and administrative handling of specialist medical care.
In the near future, various services will added to the Hospital ID. You will be notified, in due course of these additions.

How can I request a Hospital ID?

The registration desk in the Central Hall of Bronovo makes your Hospital ID for you. we ask you to bring a valid identification and health card with you.

Can I make a Hospital ID with an expired passport, ID card or driving licence?

If you do not have a valid identification, you can report this at the Registration Desk. We can only make a Hospital ID with a valid ID.

If I still have a blue registration card, or I do not have my patient ID with me and I have an accident, will I be helped?

Yes, care continues as usual.

How long is my Hospital ID valid?

The Hospital ID for adults and children, from one year and older, is valid for five years. The hospital ID for children under one year is valid for one year.
